The traffic and infrastructure level can rise with City-based Demand.

However, this system is not in place with passengers yet.

Thanks! I was just curious about some pretty big international airports that are huge in the real life and have "0" international and long-haul international routes in the game.

The traffic and infra levels itself do not affect the demand. They are just the statistical measurements on how busy/"good" the airport itself is but not related to demand at all. In City Based Demand airports themselves are in no relation to the demand anymore, they are just the gateways to serve the demand.

The demand is always there (if we think of a small airport that is in a big city).

If the player flies a lot from that airport the traffic level grows and if the airport becomes congested the infra level will be expanded.
